Calls to the office will be directed to our call center, operated by HIPAA-compliant virtual medical assistants who effectively support most patient inquiries and concerns. Rest assured that calls requiring immediate attention will be delegated to the correct departments for a timely response. To make your visit run smoothly, please fill out and print our online patient form; please bring a picture ID, your insurance card, any medications you may be taking, and any lab results or imaging of the abdomen that you may have.

Requirements for your appointment

  • We require medical records within the last 3-6 months (preferred or the last year).

  • Recent blood work within the last 3-6 months or the last year.

  • The most recent abdominal radiology exam (Ultrasound, CT scan, or MRI).

  • A visit authorization from your insurance company (e.g., a doctor’s referral and authorization or a doctor’s order indicating the diagnosis).
