To make your visit run smoothly, please fill out and print our online patient form; please bring a picture ID, your insurance card, any medications you may be taking, and any lab results or imaging of the abdomen that you may have.

Requirements for your appointment
We require medical records within the last 3-6 months (preferred or the last year).
Recent blood work within the last 3-6 months or the last year.
The most recent abdominal radiology exam (Ultrasound, CT scan, or MRI).
A visit authorization from your insurance company (e.g., a doctor’s referral and authorization or a doctor’s order indicating the diagnosis).
